Add up the yes answers in each category and multiply the sum (maximum of 3) by the rank you assigned to that category. Place the answer under the category score. When you are done calculating the score for each category, add up all 6 category scores. The maximum total score is 63.

This facility is very well suited to your needs based on the qualities and attributes that are important to you in a home. Most, if not all, of the amenities and services that you are looking for are found at this residence.
56 to 50
This facility does not have all of the qualities and attributes that are important to you. Look over the questions in which you answered no and ask yourself how important these issues are to you. Are they things that would negatively impact the elders quality of life? Would these amenities and services enrich his or her life if they were available? If so, you may want to keep looking. There are a lot of good senior living residences that may be better suited to your needs.
Less than 50
This facility may not be the place for your parent or family member. Many of the qualities and attributes that you are looking for in a home are not present. Keep looking; there may be residences in a similar price range that are better suited to your needs.
