Certified Care Management |

Certified Care Management helps older adults and their families find workable solutions to the challenges of aging. Their staff is comprised of professional in social work, nursing, and counseling. Available services include: assessment of current and future needs, identification of workable options, design and implementation of plans of care, monitoring ongoing supports, patient advocacy, cost estimation and containment, coordination of multidisciplinary care, relief for and communication with families and caregivers. Certified Care Management can also serve as court appointed Guardians and Conservators. |
